Community Health and Wellness Collective Services
The Collective provides comprehensive services designed to improve health outcomes and advance health equity. Our integrated model brings together health and wellness programming, community health worker services, education, aging services, advocacy, and community-based research to address the social, environmental, and systemic factors that shape well-being.
Through culturally responsive, evidence-based strategies, trusted outreach, care coordination, and strong community partnerships, we deliver accessible support that strengthens individuals, families, and organizations across the communities we serve.
Community Health Worker Services
Our community health workers help people across Western North Carolina, with a primary focus in Buncombe County. Individuals can reach out directly to our CHWs, or access support through partner programs in the community. Services include:
Education on managing chronic conditions
Guidance navigating healthcare and social services
One-on-one support to remove barriers to wellness
Connecting families to local resources and programs
By combining direct support with community programs, CHWs make it easier for everyone to stay healthy and thrive.
Health & Wellness Programming
Helping people across Western North Carolina stay healthy and well through programs in fitness, nutrition, disease prevention, and support that’s accessible to everyone.
Aging Services
Supporting older adults in Western North Carolina with programs and resources that help them stay healthy, independent, and connected.
Advocacy
Working to improve health equity by shaping policies, raising awareness, and amplifying community voices.
Community Research
Our Community Research initiatives help us understand the health needs and priorities of Western North Carolina residents. We collect and analyze data to identify gaps in services, track health trends, and evaluate the impact of our programs. By partnering with local organizations and community members, we ensure our research reflects real experiences and drives solutions that improve well-being. The insights we gain guide our programs, inform advocacy, and help build healthier, more equitable communities.
Education
Our Education programs help people across Western North Carolina learn the skills and knowledge they need to stay healthy and thrive. Programs are offered directly and through partner organizations, and include:
Diabetes education and support – managing blood sugar, preventing complications, and building healthy routines
Nutrition and healthy lifestyle guidance – cooking, meal planning, and fitness strategies
Housing and social support resources – connecting individuals to safe, stable housing and community assistance programs
Health literacy and skill-building workshops – understanding medications, managing chronic conditions, and navigating healthcare systems
Youth and family education – programs designed to support children, parents, and caregivers in building healthy habits
By combining direct learning opportunities with community partnerships, our Education programs give people the tools they need to improve their health and well-being.

Who We Serve
At The Collective, we support people across Western North Carolina, with a focus in Buncombe County. Our programs are designed to help individuals, families, and communities overcome barriers to health and wellness, including access to healthcare, housing support, and other community resources.
